Each application at Pembroke Hill requires supporting documentation. For your convenience, these documents are segmented by the grade to which you are applying. To download these required forms, click on the appropriate grade level box to the right.
 
These documents must be submitted to the Office of Admissions before an admissions decision can be reached. The first rounds of admissions decisions were completed January 11, 2017 (Upper and Middle School) and February 15, 2017 (Early Childhood and Lower School). However, we are happy to accept applications where space is available. Please contact the office of admissions to learn more about space availability.
