Required Forms

Each application at Pembroke Hill requires supporting documentation. For your convenience, these documents are segmented by the grade to which you are applying. To download these required forms, click on the appropriate grade level box to the right.
These documents must be submitted to the Office of Admissions before an admissions decision can be reached. To be considered in the first round of decisions, Middle and Upper School applicants must complete all steps of the admissions process by January 11, 2017. Early Childhood and Lower School applicants must complete all steps by February 15, 2017.
